When to apply and what to prepare

A. In case of COVID-19, Seasonal influenza
※Application should be submitted as soon as possible after diagnosis

Required for application：Any of the following documents issued when you visited a medical institution

Test results with your name, date of visit, and name of the medical institution, or Image of the test kit

Medical fee statements issued by medical institutions（Statement indicating tests performed and medications prescribed）

A drug information form showing the prescription of antiviral medication

Medical certificate with the diagnosis of school infectious disease（Documentation fee must be paid at the medical facility.）

【Example of attached image】Images with unclear name, date of visit, or name of medical institution will not be accepted.

*The applicable conditions for self examination without visiting a medical institution due to unavoidable circumstances are as follows.

Test kits (Not available for research use)

Test kit with the test date and name written on it with an oil-based ballpoint pen, etc.

Upload an image that includes all of the following 1-4.

Test kit with your name and test date written on it with an oil-based pen, etc.
Items that show the contents of the product, such as the test kit box or delivery note (Items that can be proven not to be for research use)

B. In case of School Infectious Diseases other than the above　
※Application should be submitted after obtaining a Certificate of Recovery Infectious Disease from the medical institution.

After recovery, the student shall download a Certificate of Recovery from Infectious Disease, have the medical institution fill out the certificate including “Date of onset” and “Date cured” (the form without the student’s name, the name of infectious disease, date of onset and date cured will not be accepted).

Please answer the questions from the link below, upload the image, and press the "Submit" button.

Report of School Infectious Disease

The Medical Service Center will issue the "Confirmation of Suspension Period due to Infectious Disease" and send it to the student’s Ritsumeikan e-mail address.

Please submit the issued application form promptly through manaba+R > Applicable Course > “Tutoring Collections".

However, if you receive any individual instructions given by the instructor, please follow it.

If it is difficult to do that (e.g., the student does not have the contact information of the instructor, etc.), please consult with the Manabi Station of affiliated campus (or the Administrative Office of Inter-Faculty Graduate Schools for Suzaku campus).
